Fruitland Park EZ Pay (auto pay) is a free service offered to all City of Fruitland Park utility customers. With FP-EZ Pay you will still receive your monthly utility bill with the following note: "DO NOT SEND PAYMENT- EZ PAY BANK DRAFT".The payment is automatically deducted from your bank account one business day prior to the due date (10^th).

To initiate FP-Ez Pay service, simply complete the authorization form below and attach a voided check from the U.S financial institution for the account to be used, if for any reason, the EZ-Pay is not paid by your financial institution, it will be treated as a returned check. 

 To activate EZ pay you must provide a voided check, or if you authorize payment from your saving account please have your financial institution provide the appropriate routing and bank account for this form. 

I authorize the City of Fruitland Park to initiate monthly withdrawal from my bank account at the financial institution above for my monthly utility bill. I undestand this authorization is in effect until I notifiy the City of Fruitland Park in writing that I no longer desire this servie, allowing the City reasonable time to act on my notification. I undertand and agree that the City of Fruitland Park will impose a NFS processing fee of $25.00 in the event the debit entry is not paid by my financial institution. The City of Fruitland Park reserves the right to cancel this agreement after providing thirty (30) days written notice.
